Lancar berbahasa Inggris yang dibuktikan dengan skor hasil tes 2 tahun terakhir, minimal 500 (paper-based TOEFL, institutional TOEFL) atau 180 (computer-based TOEFL) atau 64 (Internet-based TOEFL) atau 5 (IELTS) dari penyelenggara tes TOEFL atau IELTS resmi atau minimal 510 skor CEPT dari Cilacs UII. Calon yang berijazah S2/S3 dari English-speaking countries tidak diwajibkan menyertakan sertifikat TOEFL/IELTS/CEPT.
g. Indeks Prestasi Kumulatif S1/dokter serendah-rendahnya 3,0 (tiga koma nol) dan S2/S3 serendah-rendahnya 3,5 (tiga koma lima) untuk lulusan perguruan tinggi dalam negeri dan 3,3 (tiga koma tiga) untuk lulusan luar negeri atau dokter spesialis dengan skala 4,0 (empat koma nol). IPK yang digunakan adalah IPK pada jenjang pendidikan terakhir berdasarkan ijazah yang dimiliki.
h. Bagi pelamar yang sudah berpendidikan S2 tidak diperkenkan melamar dengan kualifikasi pendidikan S1.
